About this grant

Supports the design, implementation, and evaluation of innovative, replicable, and scalable trauma system models to strengthen trauma system resilience. Recipients carry out at least two of six defined activities to improve Emergency Medical Services (EMS) coordination, patient transfer mechanisms, resource tracking, data management, evidence-based practices, and research capacity. The program aims to improve patient outcomes and increase access to trauma care for medically underserved populations.

$300,000 to $500,000 per recipient per year (first year guaranteed minimum $300,000)
Funding for up to three fiscal years (2026–2028), subject to availability
Technical support and oversight from ASPR Program Officer

Eligibility

Must be a State or consortia of States, an Indian Tribe or Tribal organization, a consortium of level I, II, or III trauma centers, or a consortium/partnership of nonprofit Indian Health Service, Indian Tribal, and urban Indian trauma centers
Must have an active SAM.gov account with a Unique Entity Identifier (UEI)
Must have an active Grants.gov registration
State, consortia of State applicants, and other State entities must contribute $1 for every $3 awarded in federal funds (match requirement)
Recipients must conduct at least two of the six defined program activities
Must submit a final report no later than 90 days after completion of a pilot project
Must comply with cooperative agreement administrative requirements including progress reports and performance measures data
