About this grant

Supports community-based organizations providing alternatives to incarceration and direct community investments across Los Angeles County. Funded programs span behavioral health, harm reduction, housing stability, reentry support, and crisis diversion services for justice-impacted individuals. This is Year 5 of the Care First Community Investment (CFCI) initiative, administered by Heluna Health for Strategies 1 and 4.

What you get

Grant awards ranging from $50,000 to $4,956,500 depending on program area (36-month contract)
Contract initiation payment within 45 calendar days of contract start
Quarterly disbursements upon completion of deliverables and milestones
Technical assistance for declined organizations to strengthen future applications
Contract start on or around October 2026 through October 2029

How to apply

Online application submitted through the JCOD CFCI Y5 Care Grant portal
Attachment #1: Most recent income tax return (2024 or later; IRS Form 990, 990N, 990EZ, 990 postcard, 8879-TE, 568, or 1120-S)
Attachment #2: Proof of nonprofit 501(c)(3) status, business entity documentation, or fiscal sponsor documentation
Attachment #3: Certificate of Insurance, insurance cost quote, or attestation letter
Attachment #4: Detailed line-item budget using provided Budget Template (.xlsx only)
Attachment #5: Two years of recent financial statements (audited preferred, optional)
Attachment #6: Business license or tax registration certificate

Eligibility

Must have provided services within Los Angeles County for at least 2 years prior to application date
Must be a 501(c)(3) nonprofit, federally recognized tribe, business entity (LLC or professional corp), or fiscally sponsored organization
Must be registered with the California Secretary of State in Good Standing for at least one year prior to application deadline
Must be registered to do business in Los Angeles County or City of Los Angeles at the location where services will be provided
Must not have had CFCI grant funding or LA County contract terminated for cause
Funding may not go to projects that benefit Year 5 TPAs (Amity Foundation, Community Partners, Heluna Health, United Way)
No funds may be used for law enforcement purposes
Organizational headquarters must be within the County of Los Angeles
