Grant for Community Care Hubs Expanding Falls Prevention Programs

Funds community care hubs to develop and expand falls prevention referral pathways that connect fall risk screening with community-based evidence-based programs for older adults. Recipients will build clinical-community partnerships, scale evidence-based workshops through implementation site networks, and engage aging and disability network organizations in service delivery and regional expansion. The initiative is designed to strengthen connections between healthcare providers and community-based organizations serving older adults.
