About this grant

Funds outreach, retention assistance, and access/utilization support for Medi-Cal members affected by federal eligibility changes, including H.R. 1 work requirements and UIS managed care transitions. Community health centers and regional consortia may apply alone or lead joint applications with CHC and/or community-based organization subcontractors. Two complementary programs are available under a shared application architecture: the H.R. 1 Navigators for Clinics Program and the UIS Managed Care to Fee-for-Service Transition Program.

$150,000–$500,000 award per entity
Up to 10% admin fee for joint applications
Quarterly reimbursement based on deliverables
Project period: January–December 2027
Funding for retention application assistance, direct outreach, and access/utilization support

Online application via Smartsheet (single-session submission)
Subcontractor Excel spreadsheet (one tab per organization delivering direct services)
Goals and outcomes proportional to award and program population
Quarterly reporting on progress across three funded measures

Eligibility

Lead applicant must be a community health center (CHC) for H.R. 1 program, or CHC or consortium for UIS program
Every applicant — lead and each subcontractor — must have previously participated in the AB 102 or SB 154 Navigator Projects
Subcontractors must be CHC and/or community-based organization (CBO) under a health center lead
Regional consortia may not apply alone but may lead a joint application for the UIS program
Application must be completed and submitted in a single session — no partial submission or save-and-resume
