About this grant

Funds data-informed, place-based outreach and behavioral health engagement within designated high-need catchment areas in Washington, DC. Grantees provide street outreach, overdose prevention, resource navigation, warm handoffs, and coordinated linkage to treatment and community services for individuals experiencing or at risk for behavioral health and substance use-related needs. The initiative targets three geographic catchment areas across Wards 7, 8, and central/north-central DC.

What you get

Up to $250,000 per award
Up to three (3) awards available totaling $750,000
Performance period December 1, 2026 through September 30, 2027 (base year)
Potential for up to four (4) option years of continued funding based on performance

Eligibility

Must be a District of Columbia nonprofit (501(c)(3)) community-based organization
Must have a UEI number, Tax ID, and be registered in the federal System for Award Management (SAM)
Minimum of five (5) years of experience providing data-informed, place-based outreach in high-need catchment areas
Minimum of five (5) years of experience deploying staff across multiple locations and corridors
Minimum of five (5) years of experience maintaining reliable staffing and supervision
Minimum of five (5) years of experience maintaining fiscal and administrative systems to manage District grant funds
Individuals are not eligible to apply or receive funding
