About this grant

Eligibility

Must be a 501(c)(3) organization
Must provide healthcare services to women, children, and the elderly in the greater Phoenix community
Must improve long-term health outcomes for individuals and the community
Must impact the underserved
Must demonstrate a strong community need
Must promote partnerships and collaborations within the community
Category A and C applicants must have a minimum annual budget of $1 million
Previous grant recipients must submit a Final Grant Usage Report by February 1st to be considered in the next cycle
