About this grant

Supports Kansas community health coalitions in coordinating multi-sector efforts to improve active living, healthy eating, behavioral health, and commercial tobacco prevention across communities. Funded coalitions work across five social determinants of health pathways and receive technical assistance to implement evidence-based packages. This two-year coordination grant helps communities prepare for additional non-competitive implementation grant funding.

How to apply

Letter of Intent (LOI) due August 21, 2026 by 4 p.m. CT
Completed application (link sent only to coalitions that submitted LOI)
Demonstrated commitment of key stakeholders across all focus areas
Evidence of active coalition with written records (meeting minutes, bylaws, fiscal oversight, etc.)
Identification of coalition coordinator and leadership team
Description of populations of focus and health equity approach
Organizational ability to administer and support full grant implementation

Eligibility

Must carry out work through an existing, active community health coalition
Coalition must be registered through Elevate Kansas (elevatekansas.org)
Coalition must be a broad, multi-sector group that meets regularly to improve health and wellness outcomes
Must commit to using a health equity lens for all grant strategies
Must identify a coordinator position (typically full-time, funded at no less than half-time by grant)
Pathways coordinator should live in the community they are serving
Must select one package from each of the five community pathways
Must send a team of three to five coalition members to at least one annual grantee event per year
