About this grant

Supports comprehensive core medical and support services for people living with or vulnerable to HIV in Washington, DC through a status-neutral care continuum model. Funded organizations implement coordinated prevention, linkage, treatment initiation, adherence, and retention services across the HIV care continuum. The program emphasizes rapid ART and PrEP initiation, navigation services, and person-centered care regardless of HIV status.

What you get

Annual award between $350,000 and $650,000
Up to five 12-month budget periods (April 1, 2027 – March 31, 2032)
Two awards anticipated from $1,000,000 total available funding
Capacity building support through HAHSTA's Integrated Assessment Tool (IAT)
Tailored training and capacity building plans based on assessment results

How to apply

Certificate of Clean Hands dated within 60 days of application deadline
Current business license or certificate of licensure
Current certificate of insurance and copy of cyber liability policy
IRS tax-exempt determination letter and IRS 990 form (nonprofits only)
Current list of Board of Directors on letterhead signed by a certified board official
Project narrative, budget table, and budget justification
Work plan, organizational chart, risk self-assessment, and scope of services
Monitoring, evaluation, and learning (MEL) plan

Eligibility

Must be a not-for-profit health or support service provider, or not-for-profit university
For-profit entities eligible only if no qualifying nonprofits are willing and able to deliver services
Must be registered in the federal System for Award Management (SAM) with an active UEI number
Must be registered in DC Health's Enterprise Grants Management System (EGMS)
Must have a Tax ID number
Must provide documentation demonstrating delivery of health and social services to intended populations
Must possess fiscal capacity to track braided program budgets and expenditures separately
Must offer HIV testing services through other funding sources (not this grant)
