About this grant

Supports nonprofit organizations running programs that address social drivers of health, specifically nutrition security and stable/quality housing through wraparound and support services. Funding is available for new or existing programs, including pilot initiatives, in Utah, Idaho, Colorado, Montana, and Nevada.

What you get

In Utah and Idaho: $5,000–$100,000 per year for up to 3 years (up to $300,000 total)
In Colorado, Montana, and Nevada: $5,000–$50,000 per year for up to 2 years (up to $100,000 total)
Small equipment and materials eligible up to $5,000 total

How to apply

Full financial audit from within the last two years (including findings and auditor's letter)
501(c)(3) designation letter or comparable government designation

Eligibility

Program or service must align with at least one social driver of health priority (nutrition security or stable/quality housing)
Must have a full and complete financial audit conducted within the last two years
Must provide a copy of the organization's 501(c)(3) designation letter or comparable government designation
Must submit a completed W9
Must submit a completed Supplier Form
Funding request may not exceed 20% of the organization's annual operating budget
Projects must begin upon receipt of funding
Projects can last between 12 and 36 months
