Grant for Nonprofits Advancing Care for Older Adults

• Up to $150,000 per year over two years ($300,000 total)
• Up to 10 grants awarded under this CFP
• Annual evaluation support from Foundation staff or consultants
• Final site visit in the last quarter of the grant period

• Registration on the Foundation's application portal by October 21, 2026
• Letter of Intent submitted via application portal by October 27, 2026
• Full Proposal Narrative (Word or PDF, max 10 single-spaced pages)
• Budget submitted in Excel on JAHF's template
• Budget justification
• Project workplan
• IRS determination letter
• Auditors' reports
• CVs or resumes for Project Lead(s) and other leaders
• Letter of support from organizational leadership for Project Lead
• Artificial intelligence use acknowledgment

• Must be a US-based 501(c)(3) public charity, college or university, hospital, or health care organization
• Must not have previously received a major grant from the John A. Hartford Foundation (recipients of limited discretionary awards are eligible)
• Proposed work must primarily benefit older adults and/or family caregivers in the United States
• Proposed work must have a national or multi-site scope
• Must not engage in lobbying not permitted under IRC Section 501(c)(3) or Sections 501(h) and 4911
• Each eligible institution may submit only one proposal
• Project Lead must demonstrate appropriate time commitment and sign the proposal
• Grantee institution must commit to supporting grant-funded activities beyond the grant's termination
