About this grant

Supports New Hampshire Critical Access Hospitals, acute care hospitals, specialty hospitals, and partner organizations seeking to strengthen, transform, and sustain healthcare delivery for rural communities. Funded projects may address access to specialty and acute care services, technology modernization (cybersecurity, EMR, telehealth, AI), and financial sustainability through operational efficiency and value-based care. Applicants may propose projects at any readiness stage, from early planning through fully designed transformation programs.

Grant subaward funding (FHC anticipates awarding approximately $17.5 million total through this RFA)
Access to Transformation Technical Assistance Center (T-TAC) resources for project planning, implementation, and sustainability
Eligibility for non-competitive continuation funding in subsequent budget periods (contingent on GO-NORTH authorization and satisfactory performance)
Retroactive reimbursement of eligible pre-award costs incurred on or after March 16, 2026
Quarterly check-ins and information sharing with FHC grant management team
Coordination with GO-NORTH evaluation contractor

Financial Disclosure (audit, IRS Form 990, or Income Statement/Balance Sheet from most recently completed fiscal year)
Executed Certificate of Vote/Authority
Certificate of Insurance naming FHC as Certificate Holder
Unique Entity Identifier (UEI) Number from SAM.gov

Eligibility

Must be a New Hampshire Critical Access Hospital, licensed hospital in a rural area, licensed hospital proposing a project predominantly impacting rural NH patients, or an organization partnering with an eligible applicant
Applicants not located in New Hampshire must demonstrate direct benefit to rural NH hospitals, patients, and communities
Project must align with at least one Focus Area: Access, Technology, or Financial Sustainability
Indirect and administrative costs must not exceed 10% of total funding requested
Must not be a freestanding emergency department
Must not duplicate work already supported by GO-NORTH or a GO-NORTH HUB
Must not have participated in the design of this RFA per 2 CFR § 200.318(c)
Lead applicant must serve as fiscal agent and is limited to one application as lead for Preparatory Investments and Program Ready stages
