About this grant

Supports organizations serving as Rural Community Hub Leads within a Kentucky Area Development District to launch and operate local hubs that convene cross-sector partners. Funded activities include designing obesity and diabetes prevention and management initiatives and passing funding through to local subrecipients. The program is part of the Kentucky Rural Health Transformation Plan.

Eligibility

Must serve a rural population
Must demonstrate ability to reach rural populations across all counties in the Area Development District (ADD)
Must be capable of serving as a Rural Community Hub Lead in a Kentucky ADD
Must demonstrate sufficient capital and fiduciary capacity to cover program costs before receiving grant funds
Eligible applicants include nonprofits (with or without 501(c)(3) status), county government, city or township government, special district government, public/state-controlled institutions of higher education, private institutions of higher education, regional organizations, and for-profit organizations
