About this grant

Supports community-driven projects in rural Arkansas focused on nutrition, physical activity, chronic disease prevention, and wellness. Funded projects may include school-based health programs, food access initiatives, faith-based outreach, healthcare workforce training, and integrated care models. Eligible applicants include nonprofits, hospitals, schools, faith institutions, government agencies, and other public entities serving rural communities.

Eligibility

Must be a state government agency, employer, hospital, clinic, school, community organization, faith institution, community health worker, academic research partner, or nonprofit/public entity serving rural Arkansas
Must demonstrate sufficient organizational, operational, and financial capacity to administer grant funds in compliance with state and federal requirements including 2 CFR Part 200
Must have an active SAM.gov Unique Entity Identifier (UEI)
Project must clearly align with at least one HEART objective and present a credible plan for achieving measurable results
All Program Year 1 funds must be obligated by October 30, 2026 and spent by September 30, 2027
