Eligibility

Must be a legally organized entity in good standing capable of receiving and administering federal subaward funding
Eligible entity types include: licensed EMS agencies, nonprofit organizations, state/local/Tribal governmental entities, hospitals and health systems, FQHCs, primary care clinics, rural health clinics, and institutions of higher education
Must hold an active SAM.gov registration with a current Unique Entity Identifier (UEI) and have no active exclusions, debarments, or suspensions
Must demonstrate organizational capacity to manage federal funds in accordance with 2 CFR Part 200
Must operate in, or demonstrate capacity to provide direct services benefiting, one or more FORHP-designated rural Virginia localities listed in Appendix A
Collaborative applications involving multiple organizations are strongly encouraged; one eligible entity must serve as lead applicant
