About this grant

Funds a single statewide hub agency to deliver the Project C.A.R.E. program across all 100 North Carolina counties, supporting family caregivers and care partners of individuals living with Alzheimer's disease and related dementias. The hub agency employs six Family Consultants who provide care consultations, information and referral services, respite care vouchers, and dementia education and training. The program targets underserved, low-income, rural, and minority caregivers who are not eligible for Medicaid-funded services.

Renewable grant for up to two additional years for high-performing grantees
Monthly conference calls with NC Division of Aging
Professional training at least 2 times per fiscal year from the Division
Telephone technical assistance from the Division as needed

Eligibility

Must be a community-based human service agency, government entity, or university-based program
Prior experience serving caregivers and dementia-caregivers strongly preferred
Prior statewide coverage preferred
Must be able to employ six FTE Family Consultants within existing personnel structure
Must provide office space or remote infrastructure, IT support, and administrative capacity
Non-profit agencies must submit a 990
Must register as a Service Provider in eProcurement upon award
Must provide Certification of No Overdue Taxes (notarized) upon award
