About this grant

Supports nonprofit organizations, schools, municipalities, and other eligible entities in Vermont's Barre, Burlington, and St. Albans Health Districts to advance substance misuse prevention. Funded activities must be evidence-based and address one or more levels of the Vermont Prevention Model, including delaying initiation of substance use, preventing onset of misuse, and promoting healthy lifestyles. Priority is given to proposals serving older adults, LGBTQIA+ individuals, and residents of Franklin and Grand Isle counties.

Grant award of $5,000–$25,000 for January 1, 2027 – June 30, 2027 (6-month period)
Potential renewal funding of $10,000–$50,000 for July 1, 2027 – June 30, 2028

Subgrantee kick-off meeting and technical assistance

How to apply

Stage 1: Letter of Interest via Foundant Grant Portal by September 23, 2026
Stage 2 (invited applicants only): Full application via Foundant by November 10, 2026
Narrative sections describing project, population, experience, and evaluation plan
Project budget using provided template (up to 15% indirect costs)
Logic model outlining inputs, activities, outputs, and outcomes
Internal controls risk assessment
Affirmation of background check and mandated reporting (if relevant)

Eligibility

Must be a nonprofit organization, individual school, supervisory union/school district, social service agency, regional planning commission, college/university, municipality, or Vermont-recognized Native American Tribe
Must serve people or communities in the Barre, Burlington, and/or St. Albans Health District(s)
Individual people are not eligible to apply
Activities must be based on evidence-based, evidence-informed, and/or promising practices
Must address one or more levels of the Vermont Prevention Model
Must be grounded in the Strategic Prevention Framework process and model
Must comply with Standard Provisions for State Funding
Must fulfill quarterly subgrant reporting requirements
