About this grant

Provides small grants to nonprofit organizations, government entities, and public entities implementing projects that address community health needs or the social determinants of health. Funded projects may cover areas such as food security, mental health, physical activity, healthy aging, and basic needs. Collaborative projects are given priority consideration.

Identification of community partners and collaborative efforts
Description of expected impact
For schools: proof of central-office authorization

Eligibility

Must be a 501(c)(3) nonprofit organization, government entity, or public entity (certain 501(c)(4) organizations may also be considered)
Organization must serve residents within Owensboro Health's 18-county service area in western Kentucky and southern Indiana
Organization must have an established and implemented 100% tobacco-free policy
Organization must demonstrate a commitment to providing services without discrimination
Schools must include proof of central-office authorization with the application
501(c)(4) organizations may not use funds for lobbying, political campaign activities, fundraising expenses, or general organizational support
Project must address an identified community health need or social determinants of health
