About this grant

Supports nonprofit clinics, government entities, and tribal health organizations in planning, establishing, or operating clinical services for low-income and rural populations. Funded projects must increase or maintain access to preventative, medical, dental, or mental health primary care services for uninsured and underinsured patients. Preference is given to projects serving uninsured individuals and those demonstrating collaboration with other health care providers or community organizations.

How to apply

Online application form including organization and applicant information, project information, project abstract, narrative questions and work plan, and budget
Patient payment policy or procedure ensuring no person is denied services due to inability to pay (e.g., sliding-fee scale)
Governing Board Resolution form (required for nonprofit organizations)
Due Diligence Form (required for most applicants; Community Health Boards and Tribal Nations exempt)
Applicant Conflict of Interest Disclosure form
Budget narrative with line-item detail
Optional: Letters of support
Optional: Federally Negotiated Indirect Cost Rate documentation (if applicable)

Eligibility

Must be a nonprofit clinic, government entity operating a clinic, Indian Health Services unit, Indian tribal government operating a clinic, or a consortium of these entities
Must provide preventative, medical, dental, or mental health primary care services to low-income or rural populations
Must utilize a sliding fee scale or other procedure to ensure no person is denied services due to inability to pay
Clinics may be stationary or mobile but must not be solely for urgent or emergent care
Must disclose any upcoming mergers, acquisitions, or changes in organizational or legal standing
Must be located in or serve populations in Minnesota
