Eligible projects include health education, disease screening, fitness and wellness assessment, and applied research. Projects must serve residents in the communities served by Sauk Prairie Healthcare in Wisconsin.

Eligibility

Services must be delivered to residents of the area served by Sauk Prairie Healthcare (Black Earth, Lodi, Mazomanie, Plain, Prairie du Sac, Sauk City, Spring Green and others)
Programs must commence during 2027
Previously funded organizations that did not submit required grant reports are not eligible
Programs must fall into health, community betterment, education, disease screening, fitness/wellness, or research
Sauk Prairie Healthcare itself is excluded from applying
