• Grant award between $5,000 and $20,000
• One-year grant term
• Support for general operating or program-specific activities aligned with community violence prevention

• Name and contact information for Executive Director/CEO
• Primary contact information for grant application submission
• Copy of organization's logo (if available)
• Current operating budget (revenue and expenses)
• Previous year's operating budget (revenue and expenses)
• Program budget (if applicable)
• Photo capturing the essence of the organization or its programs (jpeg)
• Fiscal sponsorship documentation (if applicable)
• Proposal narrative responses to five questions

• Must be Black or Latine-serving and rooted in Chicago communities primarily on the west and south sides
• Work must align with at least one of three focus areas: Arts and Culture, Community Connections, or Power Building
• Previous and current fiscal year organizational budgets must be less than $1,000,000
• Must have a 501(c)(3) status or a fiscal sponsor with 501(c)(3) status
• Current grantees of other Siragusa Family Foundation grant programs are not eligible
• Funds cannot be used for lobbying, for-profit businesses, individuals, political contributions, research, capital support, or scholarships
• Only one application per organization per year
