About this grant

Provides funding to Florida-based nonprofit organizations that deliver direct services to breast cancer patients and survivors. Grants support three areas: living expenses for patients in active treatment, medical items for patients and survivors, and access to Tomosynthesis (3D) diagnostic mammograms and/or screening MRI services. Organizations may apply for one or more funding categories through separate competitive applications.

501(c)(3) determination letter or proof of eligible tax-exempt status
Documentation of direct service provision to breast cancer patients or survivors
Budget narrative

Eligibility

Must be a Florida-based 501(c)(3) organization, Florida governmental entity, Florida-based tax-exempt educational institution, or Indian tribe within the State of Florida
Organizations registered as 501(c)(3) outside Florida are eligible if a physical presence is maintained within Florida that is open and fully operating 12 months a year
All activities funded must occur in Florida
All funds must be spent in Florida at the applicant organization and any collaborating entities within Florida
Applications must be submitted in English
Separate applications must be completed for each funding opportunity if applying for more than one
