About this grant

Funds programs and services that address identified health needs or social determinants of health in a defined regional service area spanning counties in Kentucky and Indiana. Eligible recipients include 501(c)(3) organizations, government entities, public institutions, and certain 501(c)(4) organizations whose activities align with charitable health purposes. Funded projects must demonstrate a clear connection between the proposed activity and a community health need.

Documentation of 501(c)(3) or government/public entity status

Eligibility

Must serve residents in at least one of eleven specified counties in Kentucky or Indiana
Must be a 501(c)(3) organization, government entity, or public institution
501(c)(4) organizations may apply if proceeds are used exclusively for 501(c)(3) permissible purposes and not for lobbying or political activities
Programs must focus on activities or services addressing an identified health need or social determinants of health
Applicant must attend one grant information session before submitting a letter of intent
Schools and school-affiliated groups must include proof of authorization from the school's central office
Organization must demonstrate commitment to provide services without discrimination
Partisan political groups, endowments, memorials, individuals, field trips, or tours are not eligible
