Grant for Home Visiting Nonprofits Serving Pregnant People and Young Children

Supports evidence-based home visiting programs that provide family support and coaching to pregnant people and parents with children ages 0-5 in Illinois communities facing barriers to maternal and child health outcomes. Programs must operate under recognized models such as Healthy Families America, Nurse-Family Partnership, or Parents as Teachers. The program also supports innovations in home visiting for justice-involved families, newcomer families, families experiencing homelessness, and those managing depression.
