About this grant

Supports community-led, evidence-based projects that promote safe sleep practices and reduce sudden unexpected infant deaths (SUID) in South Dakota. Projects may include safe sleep education programs, community outreach campaigns, culturally tailored resources, and father-focused or tribal community initiatives. Priority is given to communities experiencing disproportionate rates of infant mortality.

Grant awards up to $40,000 per organization (primary awards)
Additional awards up to $20,000 for projects requesting lesser amounts
Technical assistance and support from SD DOH evaluation team
Quarterly progress meetings with SD DOH
Ongoing support, evaluation, and reporting assistance

Project description including goals and SMART objectives
Activity workplan with evidence sources, deliverables, and outcome measures
Sustainability plan
Detailed budget and budget justification
Project team and key staff information

Eligibility

Interventions must target South Dakota residents and communities
All proposed projects must include a safe sleep component
Communities experiencing disproportionate rates of SUID, sleep-related infant death, and infant mortality will be prioritized
Eligible organization types include 501(c)(3) public charities, private foundations, tribal entities, government/municipal entities, for-profit entities, and others
Project leads must complete the free Cribs for Kids Safe Sleep Ambassador training prior to project implementation
Must obtain SD DOH approval prior to implementing any changes to the submitted application or approved budget
