About this grant

Funds community-based organizations to provide suicide prevention services to Veterans, Service members, and their families who are at risk of suicide. Supported services include outreach, mental health screening, case management, peer support, benefits assistance, and non-clinical social determinants of health interventions. The program prioritizes underserved areas including rural communities, Tribal lands, medically underserved areas, and U.S. Territories.

What you get

Grant award up to $750,000 per award period
Funding for suicide prevention services including outreach, screening, case management, peer support, and benefits assistance
Up to $750 per participant per year for financial assistance related to employment, lethal means safety, and secure storage
Access to VA-provided screening tools including the Columbia Suicide Severity Rating Scale (C-SSRS)

Eligibility

Must be an incorporated nonprofit private institution or foundation with a governing board, a corporation wholly owned by such a nonprofit, an Indian Tribe (including Alaskan Natives), a qualifying community-based organization, or a state or local government
Must have an active registration in SAM.gov with a Unique Entity ID (UEI) number prior to applying
Must propose to serve eligible Veterans, Service members, and their families at risk of suicide
Must propose non-clinical services (except narrowly defined emergency clinical services)
Must include baseline mental health screening as a required service
Must demonstrate ability to coordinate with local VA facilities and VA Suicide Prevention Coordinators
Must not have outstanding federal obligations in arrears or unsatisfactory audit responses
Must not be in default on any previous federal assistance
