Internal procedures to ensure funds are only for collateral costs of receiving care in Connecticut

Eligibility

Must be a nonprofit organization, community-based provider, or other eligible entity
Must support individuals in financial need traveling to Connecticut for reproductive or gender-affirming health care
Must meet one of the statutory eligibility categories outlined in Section 173 of Public Act 25-168
Must submit IRS Form 990 for the most recent 3 years
Must submit IRS Form 1023 with attachments
Must submit IRS 501(c)(3) determination letter
Must submit most recent independent audit report filed with the DCP Public Charities Unit
Must provide guidelines for distribution of funds including anticipated budget
