• Completed online application submitted by October 30, 2026
• Current copy of organization's bylaws
• Board of Directors list
• Active federal Unique Entity Identifier (UEI)
• 501(c)(3) determination documentation (if applicable)

• Must operate as a 501(c)(3) nonprofit organization, local entity of government, or parent hospital with nonprofit status operating a rural county hospital
• Must be legally chartered and headquartered in Tennessee
• Must have an active federal Unique Entity Identifier (UEI)
• Must be able to provide current bylaws, Board of Directors list, and active 501(c)(3) documentation
• Hospital must be located in a rural county as defined by the Governor's Rural Task Force
• Only one application accepted per parent hospital per fiscal year
• Programs must be broadly open to the public and not exclude any individuals from participating
• Must send a letter of intent and receive confirmation before submitting application
