About this grant

Funds projects that directly improve quality of care and quality of life for nursing home residents in Rhode Island, using penalty funds collected from nursing facilities that failed to meet federal health and safety standards. Eligible projects include culture change initiatives, resident and family council support, facility improvement programs, and staff training. Projects must benefit Rhode Island nursing home residents but are not limited to in-state entities.

Completed General Application Form or Statewide Conference Application Form
Cover letter with brief overview of proposal, participating nursing homes, funding requested, and expected outcomes
Acknowledgement of Ocean State Procures system registration requirement
General Application Budget Template
Letters of support and indication of commitment from all participating nursing homes (if applying on behalf of multiple nursing homes)

Eligibility

Projects must directly benefit Rhode Island nursing home residents
Eligible applicants include certified nursing facilities, non-profit organizations, consumer advocacy organizations, resident or family councils, professional or state nursing home associations, academic institutions, and state agencies
Projects must not exceed three (3) years in duration
Projects must not involve capital improvements
Projects must not fund staff salaries or incentives
Projects must not enlarge or enhance an existing appropriation substantially the same as the intended use
No conflict of interest or appearance of conflict of interest
All applications must be approved by CMS before EOHHS can award funding
